2013/2014 Louis Stokes Health Scholars Program, USA

Louis Stokes Health Scholars Program is provided by The Congressional Black Caucus Foundation, Inc. (CBCF) and will grant up to $8,000 for students who plan to enroll or are already enrolled in a degree program at a full-time undergraduate accredited two- or four-year college, university, or technical school during the 2013-14 academic year.

Description
The Congressional Black Caucus Foundation, Inc. (CBCF) will award 10 scholarships to students who plan to enroll or are already enrolled in a degree program at a full-time undergraduate accredited two- or four-year college, university, or technical school during the 2013-14 academic year.  The Louis Stokes Health Scholars, sponsored by the UnitedHealth Foundation, program seeks to increase the number of qualified, yet underrepresented, college students entering the health workforce.  Preference will be given to students from underserved communities who demonstrate an interest to work in their own communities.  Students currently attending two-year institutions are strongly encouraged to apply.

Benefits:
Up to $8,000 for the 2013-14 academic year. CBCF will disburse up to $4,000 at the beginning of each semester, contingent on maintaining the minimum academic requirements.  All scholarships are to be used to cover all education-related expenses.

Eligibility
Applicants must be U.S. citizens or legal U.S. residents who have a minimum 3.0 GPA on a 4.0 scale. Applicants must also be currently enrolled or planning to enroll in a full-time undergraduate course of study at an accredited two- or four-year college, university, or technical school.  The anticipated degree must be in a subject that will lead to a career in a health field.  The ideal candidate is a student at a two-year institution who plans to complete a four-year degree and seek work in an underserved community.  Additionally, the candidate will demonstrate financial need.  Employees or immediate family members of UnitedHealth Group and its affiliates, CBC Members, the CBC Foundation staff, the CBC Foundation Board of Directors or the CBC Foundation Corporate Advisory Council are not eligible.

Application Procedure
To apply this scholarship you must provide the documents below:

• A completed online application form
• A personal statement essay from the student (500-1,000 words) that addresses all four (4) of the topics listed on the application in one cohesive essay.
• Two (2) letters of recommendation. One recommendation should come from a community or public service leader (i.e. church leader, community leader, volunteer organization). Recommender information (name and email address) is to be entered into the online application as requested. Please follow the instructions found in the application. Hard copy letters sent to the organization as well as letters sent to the scholarship email address will not be accepted.
• An electronic copy of detailed Federal Student Aid Report (SAR); this report comes from the US Department of Education, not your institution. It is typically between 5 or 6 pages long. This should be uploaded to the online application as a PDF document.
• A one (1) page resume listing extracurricular activities, honors, employment, community service and special skills.
• A recent photograph suitable for publication; i.e., a cap and gown picture, or other professional quality and professionally dressed photograph.

In addition applicants should include the following documents:

• Graduating high school seniors must provide proof of enrollment (such as an acceptance letter) into institution of study for 2013.
• Official sealed transcripts from all institutions attended. Freshmen level students must also submit high school transcripts.
